PATIENT COMMITMENT
What is the Patient Commitment?
At Weston's Las Olas Laser Eye Center, we stand behind your surgical distance vision results. Our intention is to help you maintain the best possible vision. We call it the Weston's Las Olas Laser Eye Center Patient Commitment – the primary benefit of this program is the assurance that if a re-treatment is needed to maintain your distance vision results, qualified patients will receive a re-treatment without charge (does not apply to TruVision patients).
Who is eligible for the Patient Commitment?
The Patient Commitment includes qualified patients who have had laser vision correction at Weston's Las Olas Laser Eye Center. Most nearsighted and astigmatic patients are eligible for this program. Even some hyperopic patients are eligible.
You are eligible if you are in good general eye health, have been treated at the center for your initial nearsightedness (myopia), astigmatism or low hyperopia (farsightedness), and have a pre-operative prescription as outlined in the eligibility guidelines. Please read the detailed guidelines for complete information (does not apply to TruVision patients).
To maintain eligibility in the Patient Commitment program, you need to return annually to your eye doctor for an eye exam. If you do not currently have an eye doctor we will be happy to recommend one to you as these exams are not performed at the center.
What if I am not eligible for the Patient Commitment?
If it is within 12 months of your original procedure and a re-treatment is medically advisable, patients who are not eligible for the Patient Commitment (see guidelines) may receive a re-treatment at no charge.
If it is after 12 months of your original procedure and it is medically advisable, patients who are not eligible for the Patient Commitment ( see guidelines) will be responsible for the following fees: (a) eye facility fee; (b) laser royalty fee, which is determined by the laser manufacturer; (c) fee, for pre and post-operative care, determined by your eye doctor and paid directly to the eye doctor by the patient.
How do I enroll in the Patient Commitment Program?
Patients are automatically enrolled in the Patient Commitment program upon completion of laser vision correction treatment, subject to the guidelines. A copy of the guidelines will be provided to all patients prior to their procedure. Patients must sign an acknowledgement form, stating that they understand the conditions of the program.
